To provide effective care matching and clinical navigation, we must collect information regarding the individual requiring care. This includes:

  • Identity Data: Full names, dates of birth, and relationships to the person filling out the form.
  • Contact Data: Email addresses, phone numbers, and home addresses.
  • Special Category Data (Health & Clinical Data): Physical and mental health diagnoses, mobility limitations, medication requirements, care preferences, and social care assessments.
  • Financial Data: Broad funding status (e.g., self-funding or local authority funded) to ensure we match you with viable care providers.

3. How We Collect Your Data

We collect data through:

  • Secure online intake forms (hosted via our encrypted Google Workspace environment).
  • Direct phone consultations, emails, or video calls with family members and NHS hospital discharge teams.

4. How and Why We Use Your Data

We will only use your personal data when the law allows us to. Most commonly, we use your data under the following legal bases:

  • Explicit Consent: You (or an authorised family member/legal proxy) give us clear, affirmative consent to process health data to find care placements.
  • Legitimate Interests: To coordinate effectively between families, hospitals, and social care providers.

5. Who We Share Your Data With

We act strictly as a care navigation and placement broker. To successfully transition an individual into adult social care, we must share relevant profiles with third parties:

  • Vetted Social Care Providers: Residential homes, nursing homes, and domiciliary care agencies that you choose to explore.
  • NHS & Local Authorities: Hospital discharge teams or social workers involved in coordinating the immediate transfer of care.

We never sell your data to third parties. All data transfers to care providers are conducted via secure, encrypted channels.

  • Data Residency: Your data is processed and stored exclusively within secure UK and European Economic Area (EEA) data centres.
  • Infrastructure Security: Our data intake workflows utilize an upgraded, corporate Google Workspace framework equipped with enterprise-grade encryption at rest and in transit, protected by strict multi-factor authentication (2FA).

7. Data Retention

We will only retain your personal data for as long as necessary to fulfil the purposes we collected it for, including satisfying any legal, accounting, or reporting requirements. Typically, inactive client placement files are securely retained for 8 years following a successful care transition in accordance with NHS Records Management guidelines, or permanently deleted within 6 months for uncompleted inquiries
